Parents are required to read the following prior to signing the Acknowledgement form on the Enrollment Checklist:

• Mountain Pathways’ Family Handbook

• Discipline Form

• Summary of NC Child Care Law (pdf)

• Primary/Pre-Primary Classrooms’ Toileting Policy

• Shaken Baby Prevention Policy

Print, sign and return to school:

• Child’s Medical Form (pdf)
  Must be signed by physician and accompany printout of immunization records from physician.

Fill out ONLINE FORMS:

• Readiness for Entry into Primary Class

  Required for rising or entering Primary Classrooms (3-6 years)
